Our fellow showed a case last week. I think it is interesting. Send here as a chinese new year gift for some ones interested.

about 50 y women with headach, CSF cytology exam. paitent has no previous malignant history.

图中异常细胞大小比较一致，比小淋巴细胞明显增大，大部分核偏位，似见核仁。倾向恶性肿瘤。

CFS中查见重度异型细胞，左上图看可疑有扎堆线性排列倾向伴细胞核偏一端（细胞具极性），右上图看两个细胞呈合胞状态；图中异型细胞似乎能见到核仁。

这些异型细胞作为CFS中的“异物”，基本就可考虑是转移性的恶性肿瘤。

50岁的女性，癌、淋巴瘤都可能长。结合图像，考虑为转移性腺癌。建议：

查乳腺、肺、卵巢、消化道；

脑中原发肿瘤常规要排查；

查肿瘤标志物（血+CSF）CEA、CA125、CA19-9；

流式细胞仪查CK7、LCA 。

图中显示细胞浆淡蓝色半透明状，核卵圆形，偏位，有核周空晕，一个大的中位核仁，可见双核。考虑

1、淋巴造血疾病侵犯脑膜，急性淋巴细胞性白血病最常侵犯脑膜，但此例像浆细胞肿瘤，年龄也较大，但细胞比背景淋巴细胞大太多了，有点不支持。HL？不知有无颅骨或椎体病变？血涂片及骨髓如何？也许排除粒细胞肿瘤。最后一图，胞浆突起很长，网状细胞？

2、腺癌转移？建议查甲状腺、乳腺、肺、卵巢、消化道。

3、大核仁，生殖细胞肿瘤？但年龄有点大，常在中线部位脑实质内。

4、恶黑？可能性小。

If it is multiple choice question, which one will you pick?

The best interpretation of the CSF findings is:

A. Negative, reactive monocytes

B. Atypical cells present

C. Suspicious for malignancy

D. Positive for malignant cells, epithelial vs. non-epithelial malignancy

E. Positive for malignant cells, adenocarcinoma

Some of those cells with microvilli on the top of the cells, consistent with normal choroid plexus cells, This patient is too old for a choroid plexus tumor.

I favor a benign normal finding.

Most people think it is at least suspicious for malignancy. Patient had complete body exam and imaging study and did not find the lesion of other organs. Also blood work was normal. Clinicians asked pathologists if there was some way to help to make the final dx. There was no more CSF left. What is your suggestion?
