Most of those cells in your photos are "mast cells". Mast cells is frequently seen in leiomyoma, both uterine or extra-uterine origin. When mast cell-rich, especially when mast cells are reactive looking, it can cause problem morphologically like what we are seen in this case. Interestingly, these mast cells dramatically reduced in uterine leiomyosarcoma. For Gastrointestinal and esophageal leiomyoma, please be ware of mast-cell-rich leiomyoma since it can be confused with gastrointestinal stromal tumor. Even more confused, these mast cells are often positive for CD117. Therefore, when you have a suspicious GIST, but your IHC profile is confusing you, such as CD117 +, SMA+, desmin +, but CD34 negative, you should make sure to exclude a mast cell-rich leiomyoma.
