This is a case of acute erythroleukemia, AML-M6.
From the microphotos, particularly #2 and #3, we can see several large atypical neoplastic cells with basophilic granular cytoplasm and large multiple prominent nucleoli. Picture #6 shows a large megaloblastoid erythroid precursor, evidence of dysplasia, although the picture is less optimal. The section shows sheet of these large cells seen on the aspirate, which is very evenly distributed. This is also a good indication of a marrow disease rather than a metastatic disease。 Patient had increased PSA, but prostate carcinoma will not have the basophilic granular cytoplasm and bi-nucleated cells.
PAS stain showed block type positivity in the blasts.
