小叶原位癌,图4图5的大导管是不是导管原位癌。

有可能是导管癌

 再补充IHC：

34βE12（+）

淋巴结有（1/22个）转移

没有看到明显浸润性的区域！！

我的报告：乳腺小叶原位癌（E-ca为阴性，34βE12为阳性）伴早期浸润，部分区域累及导管（因为图4、5的管壁上皮E-ca为阳性）！！

妥否？

 1. If E-cadherin stain was negative, the tumor should be a lobular lesion.

2. One of 22 lymph node was positive. The chance of positive axillary LN is very low if the primary tumor is LCIS only.

3. 我的报告：乳腺小叶原位癌（E-ca为阴性，34βE12为阳性）伴早期浸润.

Myoepithelial marker stains are necessary for this case.

4. Pathologists need to figure out it is carcinoma in situ only vs 伴早期浸润 vs more invasion for the case as above.

 在我们这级医院发原位癌是不是风险大了，发小叶癌可以吗？

 以前唐平老师曾讲过DCIS with microinvision： DCIS extend into adjacent tissue with focus 0.1cm or less.

本例E-ca为阴性，34βE12为阳性，诊断小叶原位癌成立。但既然没有看到明显浸润性区域，如何诊断伴早期浸润？而且淋巴结有转移，如何解释？

请cqzhao老师给讲讲早期浸润的诊断。
