便秘有关吗  希望老师都各抒己见

 我猜测便秘可能是类癌综合征的一种表现。

以前在网上见过一例小肠类癌转移到宫颈，现在怎么也找不到了，可惜啦。

 乳腺情况？

肿瘤组织排列成巢片、条索及腺样结构，间质也有纤维化，细胞看不清楚。这样的图象正常可能首先考虑癌（无论原发还是转移），既然楼主归为“误诊病例”，莫非是性索间质肿瘤，GCTs？

 　现在公布免疫病理结果：NSE+，CD56－，Syn+，CgA+，CDX2－，CA19-9－，B-Catenin－，CK+，CKl+，AE1/AE3+，CA125－，CK7－，CK20－，inhibin－，CD99－，S-100－。

　最终诊断：（卵巢）类癌。

　参考张建民——卵巢病理学：顽固性便秘是类癌综合征之一，可能系类癌分泌肽类物质抑制肠蠕动所致，卵巢病灶切除后，便秘症状消失有助于卵巢原发性类癌的诊断。

Morphologically insular growth pattern. DDx include carcinoid tumor and metastatic adenocarcinoma before considering other primary ovarian tumors. For carcinoid tumor, most common are secondary tumor metastasized from other source such as GI. Primary carcinoid is often associated with teratoma. I will make sure this tumor is well sampled and do not have any teratoma elements. Immunostain for neuroendocrine markers should be applied. I will also apply CK7, CK20 and CDX2 to exclude adenocarcinoma from gastrointestinal tract and other sites such as breast.

   形态学表现为岛状的生长方式，鉴别诊断首先考虑类癌和转移性腺癌。对于类癌而言，通常是从胃肠道等处转移而来的，卵巢原发性类癌往往伴有畸胎瘤，我想确认有没有畸胎瘤成分。免疫组化：除神经内分泌标记以外，还要做CK7, CK20 和 CDX2以排除胃肠道和乳腺等来源的腺癌。（天天田田译）
