就凭这几幅图不敢苟同,没看出细胞异形性，也没见浸润。

良性粘液性肿瘤,为何不可以形成粘液湖中的上皮细胞团?

那应该不是恶性粘液性肿瘤的专利，

完全有可能嘛。

 1.粘液性囊肿病；

2.粘液性囊腺癌。

1. 乳腺粘液性囊肿病

2.黏液性囊腺癌

 This is true difficult case especially based a few photos. Differential diagnosis includes benign mucocele-like lesion and mucinous carcinoma.

Focal cystic wall lined with benign looking epithelial cells with associated myoepithelial cells, hemosiderin-macrophages (suggestive rupture of the cysts), detached clusters of epithelium within the mucin (often present in mucinous carcinoma, but they have benign looking in this case). Basically my feeling is that this is  mucocele-like benign lesion. However I cannot make the diagnosis only based on the above photos in true case. I have to read all true glass slides with microscopy for diagnosis.

So above just for your reference.

 俺想到3个病要鉴别：

1、粘液性囊肿病：应少有漂浮细胞团，细胞团存在肌上皮。

2、粘液型导管内癌：也是单一细胞。

3、粘液性囊腺癌：多是高柱状粘液上皮，可漂浮腺、乳头等，无肌上皮，能见间质浸润。

本例高龄患者，囊肿2.5厘米，周围没有原位癌，囊衬高柱状细胞，有一定异型，漂浮细胞团，有无肌上皮要看IHC，间质浸润没看到。

做肌上皮标记吧，排除了粘液性导管内癌和粘液性囊腺癌，再考虑粘液性囊肿病。
