intervillous trophoblasts :绒毛间滋养细胞?似乎不如绒毛外滋养细胞更确切。绒毛外的滋养细胞几乎全部由IT(intermediate trophoblast)组成,它浸润在蜕膜,子宫肌壁间和胎盘部位的螺旋动脉。根据IT所在部位的不同又将其分为绒毛IT(位于早期胎盘的绒毛柱)、种植部位细胞以及绒毛膜型IT。无论完全或部分性葡萄胎,还是流产绒毛退变水肿(也有称水肿性流产),IT均表达p57. 但完全性葡萄胎的绒毛上的滋养细胞阴性,而部分性葡萄胎的绒毛及流产后水肿的绒毛均为阳性。所以,p57无法用以鉴别流产后水肿绒毛和部分性葡萄胎。

Sorry i feel different from above experts. I feel i cannot exclude complete mole. There is mild trophoblastic proliferation in some villi and some pictures show cistern-like spaces. Although not sure, it seems that there are no blood vessels in the stroma and i don't see nucleated red blood cells (fetal tissue). Nowaday with HCG monitoring and ultrasound, complete moles can present in very early stage before classic features develop. p57 is a very powerful tool to diagnose complete moles. If we are suspecting partial moles, we do chromosomal analysis.
I admit we don't see molar pregnancy here as much as in China. Maybe i am overworried.
