前列腺良性增生背景，镜下发现一小灶（图3为全部病灶）下图病变，可见核分裂。免疫组化依次是PSA（-）、P504S（-）、34BE12（+++）、P63（+++）。

考虑：前列腺基底细胞增生？尿路上皮病变？

 If you take a good look of pic 1 and 2, you will appreciate a layer of epithelial cells toward the lumen.  All the multiple layers of cells underneath the top layer are basal cells.  Usually basal cells have pale or clear cytoplasm, while epithelial cells have pink cytoplasm. Whenever you have a prostate,  try to appreciate features of epithelial cells and basal cells.  After a while, you probably will reduce the cases for immunostains.  Another point; whenever you see many cell layers and complex  glands as seen in this case, probably it is not cancer.  Remember, prostate cancer most time has simplified glands.
