Gleason 3+3. By comparing with adjacent normal glands, if there is nuclear enlargement and prominent nucleoli, a definitive diagnosis may be made. Immunostains for basal cells will be helpful in this situation. The specimen appears to be transurethral resection of prostate. For a 76 year old man with small focus of well differentiated prostate cancer, he may not need any additional treatment.

Relatively large area of small uniform glands in between large normal glands with luminal mucin and eosinophilic granular material; straight glands luminal surface; altered cytoplasm compared to normal large glands – highly suspicious for prostate carcinoma Gleason 3+3. By comparing with adjacent normal glands, if there is nuclear enlargement and prominent nucleoli, a definitive diagnosis may be made. Immunostains for basal cells will be helpful in this situation.
