左乳导管镜下乳管内肿物切除活检，导管内乳头状癌肿瘤

做肌上皮确定乳头状癌或不典型乳头状瘤

建议切除活检

 这例也有一些问题想问问老师们：

1、最后一张下方导管周有一圈完整的CD10阳性肌上皮细胞，上方增生的导管周围缺乏阳性细胞，如何解释？

2、增生的部分乳头轴心可见肌上皮阳性表达（P63、CD10），实性区缺乏阳性表达，如何解释？

3、CK5/6的这种表达方式有何意义？如何解释？

4、诊断癌和不典型增生在治疗上有何不同？

 Review this case again. Feel it is like a solid papillary ca even though the cytologic atypia is not marked.

Reasons:

Solid papillary growth pattern

Myoepithelial cells are absentboth within and peripherial areas.

最后一张下方导管周有一圈完整的CD10阳性肌上皮细胞，上方增生的导管周围缺乏阳性细胞，如何解释？

Above is papillary solid carcinoma (PSC)

Below: small DCIS or papilloma or UDH

 增生的部分乳头轴心可见肌上皮阳性表达（P63、CD10），实性区缺乏阳性表达，如何解释？

May stain some myofibrolast or some myoepithelial cells. Read carefully  and notice that basically they are negative

4、诊断癌和不典型增生在治疗上有何不同？ Depending on the situation, such as age, core bx or escisional bx.

Talk with your oncologists to know more details. Or check NCCN guidline

 It is not easy to make dx based on photos for this case.

I favor SPC or at least it is atypical palliloma
