Lobular carcinoma of breast may initially present as metastasis without breast symptoms. I have seen a case initially presented with ascites (Case-0390). In this lymph node, the tumor cells diffusely infiltrate the interfollicular area with relatively preserved architecture of lymph node. The tumor cells are admixed with lymphocytes, and compared with surrounding lymphocytes, they are larger with fine chromatin and small nuclei, which may mimic large cell lymphoma. However, frequent intracytoplasmic lumen is noted, and particularly in the extranodal soft tissue, there are tumor cells arranged in classic Indian file pattern.
