Teratoma without immature component is benign in prepubity patient. After pubity it is malignant no matter if it is mature or with immature component. The diagnosis should be "malignant germ cell tumor completely composed of teratoma", then mention margin status like other germ cell tumor. If the tumor or lesion in the testis is just epidermal cyst or dermoid cyst , the diagnosis is "Epidermal cyst" or "dermoid cyst" regardless of age.
