Agree that mostly it is sclerosing adenosis.

Last two photos show small glands with pink secretation, some features of microglandular hyperplasia.

If you are interested, you can stain s-100 and SMA. Epithelial cells in microglnadular hyperplasia will be positive for S-100 and myoepithelial cells will be absent.

Interested to know your results.

完全同意此例诊断为良性病变， 问题在组织学分类。
组织学与一般腺病不同：
1）有4种成分：大量平滑肌样结节、扩张的腺管、增生的末端导管、脂肪细胞；
2）部分平滑肌结节中央有导管，表明不是血管畸形；而是肌上皮细胞的肌样化生（Myoid metaplasia of myoepithelial cell)

是否可诊断为肌样错构瘤，Myoid "hamatoma", （参考文献：Rosen's Breast Pathology, 3nd Edition, p 867-869,2009）。文中指出：大多数肌样“错构瘤“是腺病瘤伴有肌上皮成分的平滑肌瘤样肌化生。遗憾的是在目前的文献中将此命名为错构瘤。 原文：This tumor also termed muscular hamatouma, is a benign proliferative lesion composed of ducts, lobules, stroma, and bands of smooth muscle cells. Most examples of this neoplasm are adenosis tumors with leimyomatous myoid metaplasia of the myoepithelial cell component. Regrettably, the designation of these tumors as hamatomas is now well entrenched in the literature.

 非常谢谢各位老师的赐教！

说实话，虽然我给了一个良性病变的报告，但是正如曹老师说的那样，这个病例组织学却与一般腺病不同，那些“大量的平滑肌样结节”，我从未见过，会不会与患者病史太长有关？动员患者拿出去会诊，上级医院给的意见为“腺病，建议随访”。

 还是不太甘心诊断为腺病。有机会寄出去再请老师看看。
