Fig.1- polypoid cystitis picture on low power; need high power to evaluate urothelium
Other figures - spindle cell stroma with edema, inflammation, granulation tissue like; but with many large pleomorphic cells - consider inflammatory/reactive process due to inflammation, postoperative spindle cell nodule (need history) . Other processes in an older man - would consider sarcomatoid carcinoma, leiomyosarcoma, and inflammatory myofibroblastic tumor.
Is there urothelial dysplasia and is there conventional urothelial carcinoma?

Agree with Dr. Shen's analysis, but suspect there is a poorly differentiated neoplasm involving the stroma. Do pan CK and other markers. It seems there are two processes going on: the first photo shows a urothelial proliferation involving only the epithelium and don't look really high grade from low power; other photos show very atypical infiltrating cells.
