乳腺癌，化疗后，有两种成分。

姓名：	×××	性别：	女	年龄：	55
标本名称：	乳腺改良根治标本
简要病史：	左乳肿物两月余，化疗2次后
肉眼检查：	一侧乳腺改良根治标本，肿物直径2cm，灰白质硬，界限不清。

免疫组化：ER(++)，PR（＋），E-cadherin（＋），P53（±），CerbB-2（＋＋），Bcl-2（＋），CK-8(+++)，CK34βE－12（＋＋），CK5/6（－）。肿块大部分为胞浆丰富嗜红染肿瘤组织，有很少量小细胞印戒样肿瘤。但是淋巴结转移的肿瘤全部为印戒样肿瘤组织6/25。

从切片中HE形态和免疫标记结果确实存在两种成分：IDC和ILC。但这是一例化疗后标本，我们无法确切了解其心辅助化疗对E-cadherin的丢失是否有影响？再者，这例病人可能会存在混合型癌的可能，只是在活检时没能反应出来，所以不好说化疗后出现两种成分！

Interesting and beautiful photos. thank for sharing. The tumors in all photos with e-cad look positive. In one of your Her2 stain photos the tumor cells in right side are positive (Score 3), but the tumor cells may be score 1 in the left side. What is E-cad stain in this area?

Did you have the breast biopsy specimen of this patient before chemotherapy?

不好意思，没有粗针穿刺病理学诊断，外科在收到细胞穿刺诊断癌后做了放疗。E-cadherin在相同的区域表达弱阳性，如下图：

It is difficult to evaluate now. Maybe Dr 笃行者 was right there are two types of tumor. But E-cad stain showed weakly positive (it is true membrane stain). Generally we will add p120 stain for some unclear case. Her2 positive (score 3) is rarely seen in classic type of lobular carcinoma. E-cad stains are similar for all tumors. I am a little confused by this case. Chemotherapy can change the morphology of the cells and also the IHC reaction. Bottomline for this case is that your diagnosis (ducal, lobular or mix) will not change the continuous treatment for this patients.

我们常规没有p120 ，可以进一些，再做些工作。

Thinking over this case. Basically all tumor cells show the same pattern of e-cad stain, weak stain. It may be a pleomorphic lobular carcinoma. Some cases of pleomorphic lobular ca can be positive for her2. Anyway just for your reference.
