I have to agree that synovial sarcoma is my favorite differential diagnosis, with peripheral PNET or extraskeletal Ewing sarcoma as a diagnosis to rule out. Obviously, immunohistochemical stains (AE1/AE3, EMA, CD99, NSE, synaptophysin, CD56) and, more specifically, FISH tests or cytogenetic analysis for specific chromosomal translocations are needed in the workup.
