AIS.
Thank HHX for excellent summary. Necleoli are usually small or inconspicuous in AIS and macronucleoli may be present in invasive adenocarcinoma. But there are always some special cases. The cells of some AIS cases can have large nucleoli and the cells of some invasive adenocacrcinoma can have small or inconspicuous necleoli. We cannot say they are invasive ca and cannot be AIS just due to the presence of prominnent nucleoli.
Fot this case mostly it is an AIS, not an invasive ca even though we cannot be 100% sure. Remember that Pap test is a screening test. The bottomline is that the patient must have tissue sampling.
