这是一例子宫内膜样腺癌的病例，宫腔病灶局限于左宫角位置，最大至今1.8cm，肌层与内膜癌之间的“海岸线”如图所示
更气人的是，该病例同时有腺肌病，临近浆膜面的异位灶中可见大部分腺体的不典型增生，如果异位灶完全是癌了又咋办？如何区分是转移灶还是异位灶的癌？请大家发表高见

如何区分是转移灶(yu should use the term invasive carcinoma, but not metastatic carcinoma  ) 还是异位灶的癌？

It is difficult for every one in daily practice.

1. Morphology observation and experience.

2. The status of endometrial carcinoma and myometrial invasion

3. CD10 stain to see stromal component surrounding the glands

If you think it is carcinoma arising from the adenomyosis, do not measure it as the depth of myometrial invasion

谢谢赵老师，问题是异位灶完全被癌取代的时候我也无法从理论上搞清是浸润来的还是异位来源的，好象只能凭我自己的感觉或临床经验以及逻辑解释，实在不行我就照实描述给临床，让他们去解决？

这是个好问题！！！

好像有篇文献讲过（记不清出处了），浸润癌癌灶周围没有CD10阳性的间质，异位灶癌变，周围有CD10阳性的间质。

刘老师，异位灶癌变为什么一定要有间质呢？当然肯定是存在“有间质”这个过程，当到了最后没有间质了，难倒就一定是浸润来的吗？
