I think this is still just a recurrent pleomorphic adenoma or mixed tumor. I do not believe this is malignant by histology, so mucoepidermoid carcinoma, adenoid cystic carcinoma and ductal adenocarcinoma are unlikely. Pleomorphic adenoma/mixed tumor may have invasive borders that make it prone to recurrence if not resected completely.

复发瘤为多结节性，肿瘤细胞主要为梭形细胞和上皮样细胞，少量粘液样间质。部分图片示实体性细条索。细胞密度较高、图11似乎有核分裂像。无明显的粘液软骨样间质和腺管样结构。4年前有手术切除史。鉴别诊断主要考虑多形性腺瘤、肌上皮瘤、肌上皮癌、低度恶性多形性腺癌。后者主要发生在口腔粘膜的小唾腺，可基本排除。复发可复习原始肿瘤组织切片确定是否为多形性腺瘤复发。一般复发性多形性腺瘤大多由于手次手术不彻底造成，形态学上的特点为少量的肌上皮细胞和大量的粘液软骨样间质，与本病例细胞非常丰富，与其不太符合。
因此，要考虑肌上皮瘤或肌上皮癌。建议1）IHC标记CK、EMA、SMA、Calponin、p63、GFAP、Ki-67、Vimentin、S-100等；
2）复习原始瘤切片，观察是否为典型的多形性腺瘤、细胞异形性、核分裂向像、包膜浸润和神经浸润。
根据形态学，首先考虑肌上皮癌。
