I also agree with most people that this is a papillary thyroid carcinoma on frozen.
I wonder why the immunostains were performed.
Can anyone who have had experience with CK19 and CD44v6 comment on this subject?
Is Ki67 helpful for a diagnosis of paillary thyroid carcinoma?

- Thank you very much for the information. As you know, frozen section diagnosis of thyroid nodule is a frequent problem.
- One of the most common issues is papillary carcinoma or not.
- Follicular hyperplasia can have focal papillary growth and papillary carcinoma have a follicular variant, this is not only difficult by frozen section, but also difficult on permanent paraffin sections.
- So far there are no reliable markers that can reliably differentiate cases that are difficult to diagnose on paraffin sections, particularly follicular variant papillary thyroid carcinoma or follicular hyperplasia/adenoma/carcinoma.
- I certainly know about the use of CK19 and Galectin-3, have not heard about CD44v6 and Ki67.
- Agree with your final statment " 诊断一定要以H&E(+cytology)为基础，免疫组化仅做参考"
