This has to be rare case - only heard about it, but do not have much experience.
- Frozen report: spindle cell neoplasm, defer to paraffin section for diagnosis
- Final diagnosis: to find evidence of epithelial differentiation is the key
1) is there any infiltrating ductal carcinoma or in situ carcinoma (history also)
2) suggestion of pan-cytokeratin stain is important
3) count mitoses, pleomorphism and necrosis
4) for specific diagnosis, either additional stain or consultation of soft tissue expert
5) to me, it is at least a low grade sarcomatoid neoplasm at this point;
6) treatment shoud be complete surgical resection with clear margin and follow up
