Pleomorphic adenoma (多形性腺瘤/良性混合瘤) is the most common salivary gland neoplasm period! When I was a cytopathology fellow, my teacher told me that if you get a case of parotid FNA and you just make a blind guess of pleomorphic adenoma without looking at the case, more than 60% of the time, you will be right. The problem is pleomorphic adenoma sometimes can be very cellular and have cytologic atypia, overdiagnosing pleomorphic adenoma as malignancy is very common. For those cases, I would like to use the term " atypical cells present, favor pleomorphic adenoma" or "atypical cells present, favor a low-grade salivary gland epithelial/myoepithelial neoplasm", this way, the surgeon will excise the tumor. I have to warn people that sometimes, the basaloid adenoma (monomorphic adenoma) and solid variant adenoid cystic carcinoma can look exactly as cellular pleomorphic adenoma. Suggesting surgery is always wise and it is safe for the patient. Thanks Hay for sharing this case.
