 腺癌，原发or转移？

 考虑叮咛腺癌，原发or转移？

 It has a neuroendocrine appearance with trabacular pattern, although the clear cytoplasm is not typical for neuroencrine tumor.  Paraganglioma?  Should mark with neuroendocrine markers.  Another remote disease comes into my mind is chordoma but the location is not typical of it (should be in cranial bone, spine or sacrum).  Very interesting case.  Waiting for the answer.

从第1张图看,表面有角化的鳞状上皮,肿瘤的确是位于外耳道且侵袭性生长,似乎是耵聍腺癌,但形态又不象,肿瘤呈梁状、巢状生长，很象副节瘤，副节瘤一般原发于中耳，考虑患者长达50年的中耳炎史伴面瘫，故我的诊断是：颈静脉鼓室副神经节瘤（穿透鼓膜达外耳）。做免疫组化CgA、syn、CK、s-100验证一下。

血窦需说不明显，但仍需除外副节瘤。

首先考虑叮呤腺癌。

 免疫组化：AE1/AE3（+），EMA（+），S-100（+）CgA（+）P63（-）HHF-35（-）CT未见中耳有确切病变，病变主要位于外耳道。
