病史？肿物大体（大小、包膜、切面）？不好猜啊，皮肤非典型纤维组织细胞瘤？高手继续！期待IHC。

Top list of malignant spindle cell tumor from the scalp in the elderly:

1. Melanoma (Positive for S100, MITF, HMB45 and Melanin A)

2. spindlce cell carcinoma (positive for Vimentin and CK)

3. atypical fibroxanthoma (no specific stainings, diagnosis of exclusion)

4. leomyosarcoma (positive for SMA)

very intetesting case and only rely on immunostains results because it is really to recognize its carcinoma nature in HE sections

最常见的老年人头皮恶性梭形细胞肿瘤：

1黑色素瘤（s100 、NITF 、HMB45和Melanin A　阳性）。

2梭形细胞癌（Vimentin 、CK阳性）。

3非典型纤维黄色瘤（无特异性染色，排除诊断）

4平滑肌肉瘤（SMA阳性）
