My opinion: CIN 2/3 with endoglandular gland involvement. 2 photos show irregular outer contour but without any desmoplastic or inflammatory response, so i am not sure about invasion. Photo 1 and 2: the nests of squamous cells don't show typical high grade nuclear features with diskeratotic or parakeratotic cells in the lumen, suspicious for ectopic prostatic tissue with squamous metaplasia in cervix. If you really want to know for sure, this lesion is PSA positive. But it doesnt matter in this case because we have for sure CIN2/3 involving glands in other photos. When you have ectopic prostatic tissue with squamous met only, be very careful not to call them dysplasia. Please note that my opinion here is only for exchange purpose. My interpretation is very limited by not able to examine the entire slides and other factors. In real life, i am quite definitive in my diagnoses. Otherwise, i will get nasty phone calls from my clinicians. |
