 最后诊断：黏液性脑膜瘤（myxoid meningioma）。

注：此型未收入WHO脑膜瘤的分类中，生物学行为属良性。

这例黏液性肿瘤界限非常清楚，我怀疑这是一例罕见的黏液性脑膜瘤，仔细检查肿瘤边缘可能发现脑膜瘤的旋涡状结构和砂砾体。根据MRI表现，我认为这不是毛细胞性星形细胞瘤，而且组织学也缺乏特征性的双相生长方式、嗜酸性颗粒小体、含有黏液的微囊或Rosenthal纤维。建议染EMA和PR。有趣的病例！

This myxoid neoplasm is very discrete and circumscribed. My suspicion is that this is a rare myxoid meningioma, and careful examination of the peripheral edge of the tumor may reveal typical meningothelial whorls and psammoma bodies. I do not think this is pilocytic astrocytoma due to MRI appearance, and the lack of characteristic biphasic growth, eosinophilic granular bodies, mucinous microcysts or Rosenthal fibers. I would suggest doing EMA and PR immunohistochemical stains. Interesting case this is!

 考虑毛细胞型星形细胞瘤。

（1）影像学上肿瘤界限非常清楚，有囊性变区域，瘤周无水肿提示低级别肿瘤；

（2）显微镜下瘤细胞排列稀疏，瘤细胞呈常梭形，黏液背景，细胞多形性和异型性不明显，未见核分裂像。

首先考虑毛细胞型星形细胞瘤。
