Final Diagnosis: sessile serrated polyp
Histology:These sections show a serrated polyp
which superficially resembles a typical hyperplastic polyp. However, in places the lesion differs from classic hyperplastic polyps by having widely dilated crypt bases, serrations extending entirely to the bottom of the crypt. Mitoses may be found in the upper half of the crypt.

Discussion:These lesions have also been referred to as sessile serrated adenomas, but this causes some consternation since the epithelium does not appear like the epithelium in classic tubular adenomas. As such, the author often adopt the term sessile serrated polyps for such cases.This differs from a so-called classic serrated adenoma as in adenomas of this type epithelium is not present at the surface of the lesion. When such large sessile lesions like this one that superficially resemble hyperplastic polyps are found, they shoud probably be managed in much the same way as clinicians presently manage tubular adenomas.(Adv Anat Pathol 2004, 11:143-9; Am J Clin Pathol 2003, 119:773-5)
