The appearance of the lesion is highly suggestive of mesenchymal lesion. But CK广(+++). That is surprising. Please show us a couple of CK stained photos.
Though renal cell carcinoma with sarcomatoid differentiation is the reasonable differential diagnosis, pan-cytokeratin is usually not positive in the sarcomatoid component. 血管平滑肌脂肪瘤 is another good differential diagnosis. But negative HMB45 and CK广(+++) are not the right immunohistochemical profile. This is a difficult case. More sampling may be helpful.
