I see a smooth muscle tumor with striking nuclear atypia. We need to do a deligent search for coagulative necrosis and mitotic count to give this tumor a precise diagnosis. May i ask Abin the english name for 奇异性leiomyoma? I cannot find the exact english match for this.
With coagulative necrosis and atypia, we need to look extremely hard for mitosis, but we do not need a exact mitotic count to call it leiomyosarcoma. Without coagulative necrosis, we need >10 mitoses/10 hpf to call it leiomyosarcoma. With nuclear atypia only and with <10mitoses/10 hpf, we call it atypical leiomyoma. Without atypia but with >15 mitoses/10 hpf, we call it mitotically active leiomyoma. To confuse even further, we have STUMP (smooth muscle tumor with uncertain malignant potential).
Is it about the same how you guys assess smooth muscle tumor!
Also, be very careful about coagulative necrosis, don't get confused with hyalinizing necrosis which is very common in benign leiomyoma.
