Non-small cell carcinoma with adenocarcinoma features.
Adenovarcinoma features include: cell clusters with glandular architecture, eccentric nuclei with prominent nucleoli, cytoplastic vacuoles.
Squamous differenciations: dense orangiophilic cytoplasm (in comparison to loose cytoplasm with vacuoles in adeno), nuclei usually very dark and irregular, nuclioli usually not apparent. SCC sometimes comes with necrotic background. SCC cells can form squamous pearls and show intercellular bridges.
Non-small cell lung cancer many times show a dominant type and another minor component. As a matter of fact, if you see cancer cells in fluid but you really cannot tell if they are adeno or SCC, you should think of lung primary, also pancreatic.
That is why here we give a diagnosis of non-small cell carcinoma without committing further if we have very limited diagnostic material such as sputum.
