• a confluent glandular pattern in which individual
glands, uninterrupted by stroma, merge and create a
cribriform pattern;
• an extensive papillary pattern; and
• an irregular infiltration of glands associated with an
altered fibroblastic stroma (desmoplastic stromal
response).

只要符合一个，即可诊断癌。

 Most areas are atypical complex hyperplasia.  A few foci show villoglandular pattern (for example photo #6) which is highly suggestive of a well diffentiated endometroid adeno.  You should submit all the endometrium together with some myometrium.  If no worse areas are identified, i would phrase like:

Small foci of well differentiated endometrial adenocarcinoma, endometroid type, arising in atypical complex hyperplasia.

 乳头状筛状结构，部分细胞异型并失去极向

未见间质浸润和浅肌层情况，

诊断考虑：宫内膜复杂型非典型增生，

 年级挺大，内膜腺体乳头状和筛状结构，腺体异型性明显。间质浸润有无不是很明确。头痛啊

不过感觉临床应该当癌处理

要考虑高分化内膜腺癌可能。我碰到一例宫底内膜腺癌浅肌层浸润。但其它部位的内膜却呈复杂型和不典型增生改变。建议多取材。
