These atypical cells are found in irregular groups, some quite crowded. The degree of cytologic atypia is moderate to marked, and I do see much cytoplasm at all. Therefore they are highly suspicious for malignancy, such as metastatic small cell carcinoma of primary pulmonay origin and, less likely, thymic carcinoma (if the lesion is mainly in the anterior superior mediastimnum) and lymphoma. If a cell block is available, immunohistochemical stains can be done to confirm it.
