I may be a minority in my conservative approach in this case - I cannot exclude the possibility that these cells seen are reactive mesothelia. Over my years of practice, I have learned to respect the degree of atypia that could be seen in reactive mesothelia. Are there more atypical cells with large nuclei and high nucleocytoplasmic ratios?

I am more interested in those background cells from the first slide. Can you focus on those background cells with signet ring like features and add couple more photos on this case?
I agree with Drs. Ma and Fu that the differential dx here is realy between reactive mesothelial cells vs mesothelioma-like carcinoma. The overall impression is probably more important that a few groups provided here.

Agree with 11 floor.
Only the first photo shows "tissue fragment" which is suggestive of carcinoma. I will never make a positive diagnosis based on these 2 groups. As i wrote in another case of chest fluid, reactive mesothelial cells sometimes have strikign atypia and brisk mitoses.
