血管母细胞瘤 

 从形态上来看，似乎首先考虑转移癌，但年龄稍小了些，做免疫组化：CK、EMA、VIM、GFAP、PR、Ki-67等排除透明细胞型脑膜瘤、胶质瘤等。

 排除一下脑肿瘤

 Looks like a metastatic tumor, need to do Ip to look for the original tumor. Due to the young age of pateint, germ cell tumors need to rule out by ip.

 我还是考虑原发，肿瘤血供丰富，细胞透亮，胶质瘤也要考虑。

Look at this case again. The last 2 high power pictures show high grade nuclei surrounding the vessels with large cytoplasm processes projecting to the vessel wall, have a feeling this may be a "Astroblastoma". A good review paper on March issue of J. Neuropathology and exp neurology. Should be put this one in DDX, so GFAP stain is helpful.

血管母细胞瘤常发生于成人小脑，大脑也可发生 ，此例血管丰富，细胞质丰富，泡沫状，有围绕血管排列特征，建议做做免疫组化。

 可归类于颅内透明细胞肿瘤，可能性大小依次为：

1.透明细胞室管膜瘤

2.转移性癌：肾脏透明细胞癌转移可能性最大；

3.透明细胞脑膜瘤。

其它象血管母细胞瘤、胶质瘤都不考虑

 考虑为转移癌 需要IHC帮助

 诊断:转移性肾透明细胞癌
