This question is difficult to answer in a few words. At the initial phase of clinical training, I believe a very important element for building a solid background is to maintain a high level of personal interest in studying all possible resources available to you. Take neuropathology as an example, identify a training institute rich in neuropathology (in terms of clinical case numbers, capable and enthusiastic teachers and research opportunities) for you to get in. Once in, work hard on learning as much and as fast as you can. If such opportunity does not exist for you in the near future, continue to build your knowledge base by studying books and journals while looking for the opportunity to present itself.
