From the standpoint of diagnostic practice, CD34 stains is almost never positive in  synovial sarcoma, but this stain in the current case is focal, weak and somewhat vague. HWCK and EMA are more sensitive markers to confirm the epithelial differentiation of SS, TLE-1 is a more specific marker for SS. The morphological appearance of this tumor is not indicated of the diagosis of the so-called malignant hemangiopericytoma/ cellular solitary fibrous tumor, one should not to make any definite diagnosis relying on IHC alone without consideration of the morphology context. CD99 is a less specific marker and can be expressed in many SBRCTs such as PNET, SS, rhabdomyosarcoma. lymphoma and small cell variant osteosarcoma.

我更倾向恶性SFT

多数情况下是滑膜肉瘤。

少数情况下是恶性SFT/血管外皮细胞瘤和PNET

支持恶性ＳＦＴ，或是SFT恶变的过程。

我的意见，没出来组化形态倾向滑膜肉瘤，不排除恶性血管外皮瘤，外周 恶性神经鞘膜瘤  PNET,

免疫 结果出来后，锁定两个诊断  1恶性SFT   2 滑膜肉瘤， 请做 BCL-2，进一步证实。如果阳性那第一诊断没问题。

再加上皮性标记和BCL-2等看看？同意SFT和SS的鉴别诊断。

形态及免疫组化更支持恶性孤立性纤维性肿瘤，一般CD34在滑膜肉瘤通常表达阴性，建议加做BCL-2和EMA。
