女29岁 左侧卵巢肿瘤大小8*6*6质硬、输卵管外观正常。右附件外观正常

B超医生考虑：勃伦纳瘤 ？
本人考虑纤维瘤 ，请各位老师点评下。

版主的良苦用心大家都懂的  我们在基层 只是看到书说的  没有真正的见到片没有报过 有时还是下不了诊断  需要老师的指点啊

纤维瘤、Brenner瘤和卵泡膜瘤大体上几乎不能区分。但镜下Brenner瘤除了纤维组织外，还有上皮成分。纤维瘤和卵泡膜瘤二者往往不太好区分。本例图片不甚清晰，但可看出来胶原很多，没有明确的卵泡膜样的细胞（胞质宽、染色淡、界限不清）。大体上也没有泛黄色。所以考虑是个纤维瘤。

Fibroma

Common; benign; arise after puberty

40% of tumors > 6 cm are associated with ascites

Gross: mean 6 cm, usually unilateral, solid, lobulated, firm, white, may have myxoid change; not associated with adhesions

Micro:

closely packed spindle cells in “feather-stitched” or storiform pattern;

may have hyaline bands and edema; no atypia

Positive stains: CD56 (not specific), WT1, ER-beta, PR, variable smooth muscle actin, occasional S100 and CD34 (Am J Surg Pathol 2008;32:884)

Molecular: trisomy 12

DD:

1.       Cellular fibroma (up to 3 mitoses/10 high power fields),

2.       Fibrosarcoma (4+ mitoses/10 HPF), massive edema,

3.       Fibromatosis,

4.    thecoma,

5.    Brenner tumor,

6.    Krukenberg tumor

有些卵泡膜细胞瘤可以表现为以纤维瘤样结构为主，多取材看看有没有卵泡膜样细胞，少数病例可能只有少量黄素化的卵泡膜细胞，最后还是要结合免疫组化结果

有啥问题 多多的发到网站上来 就是采图发送麻烦些 不过这里有很多的专家级的老师 会给大家非常好的诊断意见的

应该还有剩余标本，冰冻切片做脂肪染色，若阳性，提示卵泡膜瘤。
