诊断没有问题, 大家都是对的. 是前列腺腺泡癌伴神经浸润, GLEASON 积分 4+3=7 (有人或许认为叫3+4=7).
我要展示的是这个肿瘤已经长到前列腺外面去了, 已经在ZI肪组织里面了. 虽然有报道ZI肪组织也很罕见的可以见于前列腺里面, 但是在实际操作上如果在活检见癌在ZI肪组织里面就表明癌已经长到前列腺外面去了, 病理的分期就至少是pT3A.
All of you guys are correct. The diagnosis is prostatic acinar adenocarcinoma, Gleason score 4+3=7 with perineural invasion (some may think 3+4=7).
The point I want to make here is: the carcinoma already has extraprostatic extension (EPE) because it is already in adipose tissue. Although it has been reported that adipose tissue can be rarely seen within the prostate, for practical purpose presence of carcinoma within the adipose tissue in a prostatic biopsy is diagnostic of extraprostatic extension of carcinoma. The pathologic stage is at least pT3a.
