My feeling is that there are at least two pathological processes going on. One is acute T-cell-mediated rejection (fig.3-v1). Of course, the background mononuclear cell infiltration is seen in almost every picture. The other process is acute CNI toxicity (fig. 7,9, 10-isometric vacuolization of tubular cells).
The arterial intima appears edematous and myxoid. But I don't know how to interpret it. Sometimes I see the intimal myxoid change in implantation baseline biopsy.
