最好染一下肌上皮标记。很可能是浸润性癌。

 乳腺导管乳头状病变是一个大谱系，从普通型乳头状增生一直到浸润癌。

本例标记P63提示肌上皮完存，除非是腺-肌上皮癌，一般是导管内乳头状瘤（病）。需要观察肿瘤边缘有无浸润、有无神经累犯和/或脉管累犯，以除外腺-肌上皮癌。

Based on the p63 stain (if it represent the entire lesion), it is clear that no in vasive carcinoma is present. It is like a papilloma with sclerosis. You need to check the H&E carefully. You may consider the diagnosis of atypical papilloma if you see focal atypical epithelial proliferation. Anyway it is not a carcer. It is a segmantal mastectomy specimen, the patient needs follow up only and does not need to have more procedure.

根据p63染色（如果图能代表整个病变），可以明确没有浸润性癌。很可能是乳头状瘤伴硬化。需要仔细观察HE切片。如果见到局灶不典型上皮增生可以考虑不典型乳头状瘤。总之不是癌。本例是乳腺区段切除标本，患者仅需要随访，不需要进一步治疗。

本例非常好，说明某些病变的病理诊断非常需要免疫组化。
