Another very interesting case. I cannot believe you provide two challenge and unusual cases the same day! I basically share the differential diagnoses with Dr. Cao, but with different emphasis. Although I cannot see cytology very well, I am highly suspicious for a "juvenile granulosa cell tumor with focal lutinization", rather than a true Sertoli-Leydig cell tumor. Immunostaining with inhibin and EMA should be able to sort them out. Thanks for sharing with us!

细胞学看不清楚，但是根据图我觉得这个肿瘤是性索间质肿瘤，可能是Sertoli-leydig cell tumor
鉴别是adult granulosa cell tumor 成人颗粒细胞瘤， 不象上皮癌，病人的年龄也不太支持上皮癌（总体结构不象small cells carcinoma, hypercalcemic type)。你们是否见Leydig cells？有些结构象睾丸的未成熟小管，还有小囊样结构，这些都支持Sertoli-leydig cell tumor。
