Is this perforated?
If yes, it is a low-grade (well differentiated) mucinous adenocarcinoma.
If not, it is a boardline mucinous neoplasma (the glands in this case are relatively bland-looking. Studies have shown that many glands in the appendiceal wall are misplaced due to the extention of abundant mucin pools). However, if there is destructive invasion, it then is a WD mucinous adenocarcinoma.
