Top 2 photos show cystern formation and the lower left photo shows atypical trophoblastic proliferation. Normal/hydropic chrionic villi are also present. Also, the outerlines of the villi are scalloping instead of round-up as seen in hydropic changes. I suspect haditidiform mole, especially partial mole. Would do chromosome analysis to confirm (most partial moles are trisomy). You can look for nucleated red blood cells in blood vessels in th villi. If you find some, basically you can exclude complete mole. If you cannot exclude complete mole, you can do p57 immunostain to confirm.
It is very difficult to evaluate molar pregnancy nowaday because molar pregancies are terminated in very early stage due to very high HCG. Your HCG number helps only when you know how many weeks and days she was pregnant, and you have figured out the HCG lever was much higher than expected from her length of pregnancy.
