如果这个颈部肿块不是在颌下区也不在甲状腺的区域，将考虑汗腺来源肿瘤的可能性。我将更倾向汗腺来源。

这个肿瘤有包膜，边界清楚，而且靠近包膜的腺样组织呈推挤状。因此我考虑是良性肿瘤。肿瘤由小泡状结构互相连接呈小叶状，由两种基底样细胞组成，排列成腺泡或小管。构成小管的细胞偏大，嗜酸性胞浆。小细胞的胞核深染，排在大细胞的周边。

倾向汗腺腺瘤。

 从上述图片我考虑如下鉴别诊断：1.多形性腺瘤2.基底细胞腺瘤3.腺样囊性癌4.粘液表皮样癌

从上图未见明确包膜浸润、神经浸润，在冰冻切片时我宁愿考虑为良性以1.多形性腺瘤2.基底细胞腺瘤可能性大。

 冰冻图片很漂亮。

考虑腺样囊性癌。鉴别：基底细胞腺瘤。

细胞小、较一致，有异型性，排列成腺管状、筛状，腔内淡蓝色黏液，细胞巢周细胞无明显栅栏状。

 腺样囊性癌adenoid cystic carcinoma：肿瘤细胞排列成大小不等的团块，其间呈筛孔状、小管状。小管由2、3列细胞围成，内层为腺上皮细胞，外层为肌上皮细胞。管腔内可见粘液。

鉴别：基底细胞腺瘤、基底细胞腺癌、多形性低度恶性腺癌

I did try to the complete history intially.

We asked the surgeon where exactly the tissue was from.

The answer was "perhaps submandibular gland".

I am sure that you will agree that salivary gland histologic classification can be very difficult.

From the above discussion, some favor adenoid cystic carcinoma, others favor benign tumors.

Please consider the following: tumor edge, types of cells, true tubules or not, do we have chondromyxoid stroma, what about the cellular arrangment, pleomorphism, peripheral cellular palisading, cysts or mucus cells?
