Actually Xiaohe brought a very important topic for cervical histopathology and cytology. Sometimes atrophy can confuse you for CIN2 lesion. We all know one of the most challenge cervical cytology is atrophy from postmenopausal women. Condense nuclei with atrophic cytoplasm gives you high "N/C" appearance and mimics HSIL. Overdiagnosis of atrophy to HSIL is one of the most common mistakes seen in cervical cytology. We just had cervical screening project done in one of the rural areas in China. When we compared cytologic diagnosis, HPV results and biopsy results, we find there are some false-positive cytologic diagnoses. Among them, overdiagnosis of atrophy to HSIL is the most common one. Therefore, remember this case and thinking of cytology will prevent mistakes during your practice.
