If E-cad is true positive, it is a invasive ductal carcinoma.I do not appreciate the features of medullary ca. In fcat medullary ca is rare and many people think it is not present.  If you want to mention ductal ca with neuroendocine feature, you must demonstrate postive stains with chromogranin and/or synaptophysin positivity.
