In the US, cytotechnologists (see other topic to know who can be cytotechnologists) can release the benign Pap smears without any abnormality. If the Pap smears show reactive cells, low cellularity, inflammation, et at. The cases will be send to pathologists to review and sign out.

In other words, all abnormal Pap smears and all no gynecologic cytologic cases (FNA, exfoliative need pathologists or cytopathologists to review and sign-out after cytotechnicians screen and write their interpretation.

We know the conditions in the US are a little different from China. The term of cytotechnician may indicate different meaning in the two countries. Can you shares the situation in your hospitals? Which ways do you think are better to patient care?

        中国比较乱，干细胞的什么人都有，甚至有许多化验员当细胞病理医生用的，现在慢慢改变，我们医院过去发细胞学报告的不是医生，是细胞技师，他不是职业医生，知识少，发细胞报告非常吃力，更不要说穿刺细胞，尽管非常努力，但是因为懂得太少，工作非常不容易，如果其它医生不帮他，他根本就干不下去。

      因此让一个没有病理职业医师能力的人干细胞学的全部诊断工作，是十分危险的。如果只是初查，有问题找上级大夫，象美国那样是不错的。这点中国需要马上学习美国的现成经验。

 我觉得让经过正规培训的细胞病理技师签发无异常的宫颈脱落细胞学的报告是可以接受的。

国内的情况是，过去似乎完全不管资质，而现在只论资质不论实际的工作个水平。其实，资格与水平在很多时候似乎都是不能统一的。

国内病理医生那么少，而脱落细胞学的市场需求是非常大的，都医生队伍中的少数稀有人群病理医生来负担所以的脱落细胞学报告那得把多少病理医生累死啊。

 我觉得让经过正规培训的细胞病理技师签发无异常的宫颈脱落细胞学的报告是可以接受的。

In the US cytotechnicians can release the Pap without any abnormality, even no reactive change. Generally cytotechnicians graduate from college (most with biology major) and need one full year of cyto school study. Also they need to pass the board exam. The Pap smears interpretated by cytotech need 10% cases with quality control. In other words, 10% of Pap read by cytotech A must be reviewed by other cytotechnicians.
